Thyroid medications (levothyroxine). Levothyroxine is one of the most common medications affected by multivitamins. So iron and calcium can bind to it and form complexes that inhibit absorption and reduce the effectiveness of the drug. To prevent this, take levothyroxine in the morning on an empty stomach with water and wait at least 4 hours before taking any calcium, iron or magnesium supplements.

Blood thinners (warfarin). Most multivitamins contain vitamin K, and starting or stopping taking them suddenly may affect how warfarin works. These fluctuations in vitamin intake may reduce the effectiveness of the medication and increase the risk of bleeding.

Antibiotics (tetracyclines and fluoroquinolones). Some antibiotics can bind to minerals found in multivitamins, such as magnesium, calcium, iron and zinc. This may reduce the effectiveness of the medicine. Therefore, separate antibiotic and vitamin intake by at least two hours.

Osteoporosis medications (bisphosphonates). Alendronate (Fosamax) is very sensitive to interference with absorption. It should be taken on an empty stomach with clean water. Even small amounts of calcium, iron, or other minerals from multivitamins can make this medication less effective.

Diuretics. They affect how the kidneys process important electrolytes: calcium, magnesium, and potassium.

Antacids and proton pump inhibitors (PPIs). Long-term use of such medications may affect the absorption of vitamin B12, magnesium and iron.
