A tampon forgotten in the vagina. Its odor can be described as an extremely strong, unpleasant odor, resembling rotting garbage. Urinary tract infection. It can cause foul-smelling urine. Vitamins, medications, and foods can also change its smell. This type of urine may also indicate a kidney infection.

Diabetes. Sometimes urine may have a sweet, fruity odor. This could be a symptom of diabetes. If left untreated, diabetes can lead to blindness, kidney failure, nerve damage, heart disease and stroke.

Sinusitis. This is inflammation of the sinus tissue, which are the spaces in the bones that connect to the nose. These allergies may occur due to bacterial or viral infections. Such infections can cause bad breath.
