Particular attention is paid to the first month after discharge when the risk of complications is high. Usually there is an observation breaking that can lead to repetitive hospitalization. A digital instrument has been developed to reduce such risks – a sound assistant connected to daily patients.

The system records information about wells, symptoms, pressure and blood sugar. Patients can also ask a question to the doctor and get the answer in the next call. If the system detects deviations, the data is automatically illuminated for the doctor in the interface of the medical system. The doctor decides whether to adjust the treatment, invite the patient to an appointment or cause urgent care.

The project includes five categories of disease such as heart failure, hypertension, oncology, postoperative injuries and diabetes. The technologies used do not require access to the Internet and are suitable for patients with any digital skills.
