A case study published mid-September in the scientific journal Journal of Infection, by scientists at the University of Catania, Italy, reveals the first documented report of a co-infection by monkeypox virus (Orthopoxvirus/monkeypox), covid-19 virus (SARS-CoV-2) and AIDS virus (HIV).

The presented patient is a 36-year-old Italian man who had unprotected sex with other men during a five-day stay in Spain. Nine days after his return, he had a fever, sore throat and headache, fatigue, and enlarged lymph nodes. tested, SARS-CoV2 infection initially confirmed.

Later, he developed rashes with the formation of pustules on his face and other parts of his body. Biological samples sent to RT-PCR confirmed monkeypox infection. He also tested positive for HIV, revealing the covid-19 sub-variant to be omycron BA.5.1.

What is the significance of this monkeypox, SARS-CoV-2 and HIV coinfection?

The text is to emphasize that the importance of this study is that the symptoms of monkeypox and covid-19 can sometimes overlap, in which case a collection of anamnestics addressing the patient’s sexual habits is essential in making an accurate diagnosis. Then, for people with a history of recent travel to areas with smallpox outbreaksOropharyngeal swab should be done even if there is no skin rash.

In addition, the case made it clear that sexual intercourse could be the dominant mode of transmission. Like this, Comprehensive STI screening is recommended after diagnosis of monkeypox. Not coincidentally, given the high CD4 lymphocyte count, the patient studied tested positive for HIV-1, possibly with a recent infection.

Since the monkeypox diagnostic swab is positive after 20 days, it is estimated that the patient remains infectious for several days even after clinical remission. As this is the only known case of infection with the three viruses, there is still no evidence that this could worsen the patient’s condition. She was advised to self-isolate at home and has already started treatment for her HIV infection.
